Some rules in sports come about because the play was not originally covered in the rules. Sometimes because of lack of foresight, other times due to malevolence. Answer these questions about those times.

Due to NHL coach Roger Neilson continually adding players on the ice in a two-man disadvantage, any too many men on the ice call in the last two minutes calls for what infraction to be called instead.
In the opening game of the 1978 season, Dave Casper collected a Ken Stabler fumble after it had been batted forward 24 yards for a touchdown. Since this game, fumbles on 4th down cannot be advanced. Give either of the two names this play is generally known.
In 1980, this Baltimore Orioles manager began to list an unused starting pitcher as the DH, and then would determine who would be the DH depending on the situation when the spot came of. In the offseason, the AL passed a rule stating that the DH must bat once before being lifted if listed in the starting lineup.
On December 15, 1973, these two schools battled to an 11-6 NCAA *basketball game*, with one school refusing to leave their 2-3 zone while the other school just continually passed the ball near the half-court line. However, unlike other incidents, it took 13 years for the NCAA to add the shot clock. Name either of the schools involved in the game, both of which started with the letter 'T'.
When the infamous "pine tar" game resumed on August 18, 1983, two of the Yankees that were in the original lineup from July 24 were no longer available. As a cheeky protest, Ron Guidry played centerfield and this first baseman at second. To this day, he is the last left-handed thrower to play second base.
In 1978, NHL owner Harold Ballard was not fond of a new rule requiring names on the backs of jerseys, so he had the names in the same color as the jersey. What team was Mr. Ballard the owner for at the time?
While coaching the Carlisle Indian school, this man would occasionally have football-sized decals of footballs sewn onto the jerseys of his players. This was disallowed in 1913.
Trevor Chappell's final ball against New Zealand in a 1981 cricket match was thrown in this style to preserve a 6-run lead. The style of throwing in question was immediately disallowed in future matches.
During a Bulls-Knicks game in 1990, this player managed to catch an inbounds pass and sink a three-pointer in one-tenth of a second. Starting the next season, if the game clock showed less than three-tenths of a second on the clock, baskets could only be scored by tip-ins.
In 1987, minor league catcher Dave Bresnahan was kicked out of a game (and then released by his team) for using what object (instead of the game ball) to tag out a runner during a pickoff play.
